Parents are always concerned about fertility issues. Parents dream of having their children running around the house, but sometimes life doesn’t give us what we want. It may pull different stunts on you before it gives you what you want.

Infertile couples who have been diagnosed with fertility issues are usually presented with a range of fertility treatment options, including IUI and IVF. IVF requires that the female’s eggs be removed and allowed to fertilize with sperm in vitro, before the embryos are transferred back, whereas IUI does not require egg removal from the female’s reproductive organ nor does it require in vitro fertilization.

Intrauterine insemination (IUI) is a type of assisted reproductive technology (ART) that involves placing sperm directly into the uterus to increase the chances of fertilization.

The IUI procedure involves the following steps:

  1. Ovarian stimulation: The woman is given medications to stimulate the ovaries and produce more eggs.
  2. Sperm washing: The man’s semen is collected and the sperm are washed and concentrated.
  3. Catheter insertion: A thin, flexible catheter is inserted into the woman’s cervix and guided into the uterus.
  4. Sperm insertion: The concentrated sperm is then injected into the uterus through the catheter.

Sooner or later, every couple aspires to become parents. There’s nothing better than being able to conceive naturally whenever you start planning. If one finds it difficult to conceive naturally, it can be a nightmare. If a couple cannot conceive even after trying for more than a year, it is considered infertility. Infertility can have many causes, both in males and females. There are currently many treatment options available to help such couples. Several of them only require medicines, while others respond positively to minimal interventions such as injections, IUI, etc.

There is a subset of couples that get pregnant just by undergoing laparoscopic or hysteroscopic procedures. Almost all cases of infertility end in IVF, also known as test tube baby treatment. All of us are aware that every procedure has its limitations and the success rate is never 100%, but it is still very disheartening for us all. Even so, there are many factors that can improve the chances of a successful pregnancy.

An IVF cycle’s success rate is determined by the following factors:

1. Quantity and quality of eggs –

IVF results are very much influenced by both the quantity and quality of eggs retrieval in a cycle. There is no doubt that a higher number of eggs retrieved increases the chances of a successful pregnancy. A successful pregnancy is also determined by the quality of the eggs. The quantity and quality of eggs can be improved using many medicines such as DHEA, Coenzyme Q10, etc.

2. Sperm count and quality – 

Sperm quality and quantity are equally important factors in determining the success of an IVF cycle. A sperm is usually present in millions, so it is the quality of sperms that determines how embryos grow and develop. While planning pregnancy, avoiding smoking and drinking plays a crucial role in determining how successful the cycle will be. Often, antioxidants and minerals are recommended in order to improve the quality and potency of sperms. DNA Fragmentation Index (DFI) is a test that is associated with a reduced success rate of cycles when high. Some medicines can help to reduce DFI.

3. Embryo Transfer timing – 

In comparison to Fresh Cycle transfer or Day 2 – Day 3 embryo transfer, ultrasound-guided transfer of Day 5 embryos, known as Blastocysts, has been shown to have quite a high success rate in FET cycles. It is recommended that couples who have previously failed multiple IVF cycles first go for the ERA (Endometrial Receptivity Array) test. This tells us if the uterus is ready to accept the embryo. When the uterus is not ready, we plan the embryo transfer a bit later. As a result, many couples who had previously failed IVF cycles have been able to get pregnant in one shot.

4. Selecting the best Embryo – 

The success of an IVF cycle is also determined by the quality of the embryo. In the past, embryos were graded based on their structure. Currently, we have the option of PGS (Pre-implantation Genetic Screening) or PGD (Pre-implantation Genetic Diagnosis). These tests allow us to identify embryos with genetic abnormalities and discard them. By selecting the best embryo and transferring it, the success rate of the IVF cycle is enhanced.

5. Optimal body weight – 

During IVF, this is a factor that is often overlooked. Women who are overweight or obese have a higher risk of miscarriage and implantation failure. Weight gain is often accompanied by insulin resistance, which is responsible for the poor pregnancy outcomes. Such women are often prescribed drugs like metformin to improve the success rate of their IVF cycles. Diabetes patients are usually prescribed this medicine, but obese and PCOD patients can also benefit from it. It does not cause any major side effects in nondiabetics such as low blood sugar levels.

Discomforts During Pregnancy, along with Bowel and Bladder, problems and other conditions, such as haemorrhoid formation, incontinence, constipation and frequent urination, are the most common during pregnancy. Therefore, both a healthy diet and regular exercise are essential during Pregnancy.

Here we will discuss each problem to see how normal it is during and how to prevent it. 

Constipation

In your early pregnancy days, you will face difficulty in passing stool or feel Constipation. The hormonal changes during early pregnancy cause relaxation of the smooth muscles around the stomach. This slows down the passage of food to the intestines. As pregnancy progresses, the baby’s head also increases the pressure on the rectum.

Iron supplements can also lead to Constipation or worsen it.

Constipation can make the stool hard and lumpy.

Haemorrhoids or piles are swollen veins that form in or around the lower rectum and anus. Haemorrhoids during pregnancy can occur due to Constipation. They can be as small as the size of a pea or large as a grape. Itching and pain are the common symptoms. Bleeding can occur in severe condition, which can make the passing of stool difficult.

Haemorrhoids can be managed by:

  • Eating fibre-rich food, along with whole grains and fruits and vegetables to prevent Constipation.
  • Drinking plenty of water.
  • Icing to prevent swelling and itching.
  • Dipping the buttocks in a hot bathtub for 10 to 15 minutes every day. This can be done quickly using a Sitz bath, which is available at a medical store.
  • Not straining while passing stool.
  • Avoiding prolonged sitting, especially on the toilet seat, increase stress on the rectum.
  • Regularly changing the position to increase the circulation.
  • Pushing them inside if they start hanging.
  • Cleaning the anus properly after passing stool.
  • Patting over the painful area rather than rubbing.
  • Applying a stool softener crème over the area, as prescribed by the physician.

The woman should immediately consult a physician in case of bleeding from the area.

Frequent Urination

Frequent urination during pregnancy

An urge to pee often is common during early pregnancy. Some women may experience this symptom until the baby is born. The problem may arise due to:

  • The pressure of the growing baby’s head on the bladder.
  • Increase in the volume of blood flow.
  • Hormonal changes during pregnancy.

Frequent urination is the first sign of Pregnancy and starts around six weeks.

The urge to urinate during the night can be prevented by cutting down late evening drinks. Rocking back and forth while peeing releases pressure on the bladder, which may further reduce urination.

Frequent urination can be managed by:

  • Completely emptying the bladder.
  • Skipping a few drinks, such as alcohol, tea, coffee, or soft drinks, contain diuretics, increasing the urine output.

A physician should be consulted if there is any pain or burning sensation while passing urine or blood in the urine.

Incontinence or Urine Leakage

Another common pregnancy problem is incontinence. It occurs because the muscles of the pelvic floor relax, preparing the surrounding structures for delivery. The pregnancy hormones further increase this relaxation by making the tissues and ligaments more elastic. Once the tightness is reduced, even mild coughing, sneezing or changing the position can cause small urine leakage. Incontinence can be managed by:

  • Regularly emptying the bladder.
  • Practising Kegel’s exercises to strengthen the muscles of the pelvic floor.
  • Eating healthy food to prevent gaining excessive weight.

C-section delivery is defined as delivering a baby via a surgical opening in the mother’s lower abdominal region. It is also called C-section. It would help if you had more recovery time after a C-section delivery than standard delivery. A mother needs to be monitored Regular after cesarean delivery to check her vitals and problems, such as mood swings, vaginal discharge, swollen breasts, and many more.

Recovery following C-section

Hospital Stay

The hospital stay following C-section is usually three days; however, this duration may vary from person to person. You will be moved to the recovery room for further monitoring. Immediately after the surgery, 

Day 1

Your provider or nurse will massage your womb so that it returns to its original size. The health care provider will encourage weight-bearing and walking. In case of pain, you may be given analgesics to relieve pain. Walking is essential to prevent any complication in the stomach. You will be given pads to manage the bleeding.

Day 2

On this day, your catheter will be removed. You will be encouraged to walk more to avoid any more complications.

Day 3

This can be your discharge day. Your provider will cover your sutures with a clean, sterile bandage. You will be advised about caring for your sutures at home, along with other instructions on feeding the newborn, avoiding sexual activity, regular changing of pads, and douching until the follow-up. The incision typically heals within a month. Contact your provider if you feel pain at the site of the incision.

Home care

You should follow the following instructions to make your postcesarean delivery period easy and pain-free:

  1. Rest: Taking an appropriate rest is the best way to recover.
  2. Analgesics: You may have pain for 7 to 14 days after a C-section. Take NSAIDs, such as ibuprofen, in case of severe pain.
  3. Ambulation: Walking improves healing by increasing blood flow, preventing clots’ formation, and lung disorders, such as pneumonia. Take support if you cannot walk independently, and increase your pace with each passing day.
  4. Exercises: Exercises, such as swimming, are beneficial for an early recovery. However, take care of the stitches. Avoid strenuous activities, such as aerobics, bicycling, and lifting heavyweight for about a month. Do not perform squatting as it may strain the abdomen and lead to complications.
  5. Incision care: Keep the stitches and the surrounding area clean and dry. Assist your stitches with a pillow while coughing or sneezing.
  6. Take showers and change pads regularly to avoid infection. Also, do not douche or use tampons unless your provider instructs you to do so.
  7. Diet: Eating foods rich in vitamin C increases the production of collagen. Collagen is considered to be a tissue repairing protein. Foods, like nuts, are rich in omega 3-fatty acids, which reduce inflammation.
  8. Constipation: Constipation is one of the most common complications encountered during and after pregnancy. Eat a fibre-rich diet.
  9. Breastfeeding: it is important to breastfeed your child in the correct position. Follow the instructions of your provider. Always sit with your back supported. DO NOT lean forward, as it may strain your abdominal muscles.
  10. Counselling: Depression is common following delivery. Seek the help of a counsellor to prevent psychological disturbances.
